Student and Parent/Guardian Agreement and Permission
Short-term private lessons or sessions are defined as a package of four, eight, or twelve lessons that do not subscribe to a monthly plan. Advance payment for all lessons or sessions is required to reserve your lesson time on the instructor schedule.
Expiration: Lessons expire 4 months after purchase date.
Attendance Policy: 1 of 4 lessons may be rescheduled for a mutually convenient time with the instructor if a 48-hour notice is provided. However, short-term package lessons or sessions are not eligible for rescheduling, refunds, or credits if the student fails to provide at least 48-hour notice of absence.
Instructor Absence Policy: Should a teacher or therapist be unable to attend a lesson/session due to illness, emergency, or any other reason, the following measures will be taken: 1) the lesson will be rescheduled, or 2) a qualified, pre-screened substitute will be provided to conduct the lesson/session in their stead.
In the event that Music Works needs to close the building for any reason, lessons will be taught via online platforms.
Student and Parent/Guardian Agreement and Permission:
1. I have read and consent to the terms and conditions stated above.
2. I commit to paying tuition promptly and accept any financial penalties that may accrue due to my child's or my own non-payment, or for any physical or property damage caused on the premises of Music Works Northwest.
3. I will inform the Registrar of Music Works Northwest, as well as the student's instructor or Music Therapist, of any medical conditions, medications, or special medical concerns or needs that may affect the student's performance and/or attendance.
4. In case of a medical emergency, I authorize any representative of Music Works to obtain medical care for my child or me. I will bear all costs related to medical treatment and emergency transportation.
5. I permit Music Works Northwest to utilize photographs or videos of my child or me for promotional activities.
